All athletic programs at Harvest Park Middle School stress sportsmanship, fair play, and are a privilege to be part of.  The interscholastic program exists to encourage participants to achieve their maximum potential and to positively represent their school.

 

Every student is urged to participate in the athletic program.  Participation depends upon:

  • A satisfactory medical examination

  • Ability in the sport

  • 2.0 GPA with no F’s

  • Agreement to PUSD’s Code of Conduct

  • And compliance with all other rules of eligibility.

 

Physical forms, insurance waivers, and parent permission forms must be completed and filed before a student is eligible to practice or participate in athletics. All forms may be found on the school’s website. 

 

Athletic Packets must be submitted by:

Fall 

Winter

Spring 

Cross Country


Boys’ Basketball


Packets due by 8/14/20 to Mrs. Ricci by 3pm 

Girls’ Basketball


Packets due by 10/9/20 to Mrs. Ricci by 3pm 


Boys’ volleyball

Packets due by 12/17/20 to Mrs. Ricci by 3pm 


Wrestling

Packets due by 1/15/21 to Mrs. Ricci by 3pm 

Girls’ Volleyball 

Packets due by 2/26/21 to Mrs. Ricci by 3pm 


Golf 

Packets due by 3/5/21 to Mrs. Ricci by 3pm 


Track and Field 

Packets due by 3/19/21 to Mrs. Ricci by 3pm 


Cheer (Tryouts for the next year; details to follow.) 

Students who do not have a completed athletic packet submitted to the Athletic Secretary by the above dates, will not be allowed to try-out for or compete in any sports for the specific season. 

 

Students must also have a completed physical release form before taking part in a sport. This physical must be performed by a licensed physician. The physical form is valid from one (1) year of the physician’s signature date.

 

Please note: Physicals performed by a chiropractor will no longer be accepted.

PLANNING TO DRIVE CHILDREN OTHER THAN YOUR OWN?

For those parents interested in becoming a volunteer driver, the Insurance Declaration Page with insurance coverage and expiration date must be attached to the Transportation form.
